PICO(T) Question: Structured query that includes Patient/population, Intervention, Comparison (optional), Outcome, and Time frame. Levels of Evidence (Evidence Pyramid): Ranks study designs from most to least reliable (Level I = systematic reviews/meta‑analyses of RCTs → Level VII = expert opinion). Critical Appraisal: Three questions – Is the study valid? What are the results? Are they applicable to my patients? Iowa Model: A step‑wise framework for implementing and sustaining evidence‑based practice changes in nursing. --- 📌 Must Remember Hierarchy of Evidence: I > II > III > IV > V > VI > VII. Evidence Synthesis Summarize findings across studies; note concordance or conflicts; grade overall strength. Integration & Decision‑Making Merge synthesized evidence with your clinical judgment and the patient’s values to choose an intervention. Implementation (Iowa Model) Identify trigger → assess relevance → rank priority → assemble multidisciplinary team → pilot → evaluate → scale or discard. Evaluation & Dissemination Measure outcomes (e.g., reduced infection rate, improved satisfaction). Share results via presentations, rounds, or publications. --- 🔍 Key Comparisons Quantitative vs. Qualitative Research Quantitative: Numerical data, focuses on effect size, higher placement in traditional hierarchy. Qualitative: Narratives/experiences, provides patient‑centred insight, often placed lower (Level V‑VI). Level I Evidence vs. Expert Opinion Level I: Systematic review/meta‑analysis of RCTs – highest internal validity. Expert Opinion: Based on experience, not systematic; lowest hierarchy. Framing Effect: If evidence is presented as “70% success” vs. “30% failure,” clinicians may choose differently—recognize and re‑frame objectively. --- 📍 When to Use Which Use Level I‑II evidence when the patient population and outcome closely match the study. Turn to Level III‑IV when higher‑level evidence is unavailable but the question is urgent. Rely on Qualitative findings to understand patient values, cultural considerations, or when outcomes are subjective (e.g., pain, satisfaction). Apply the Iowa Model for any systematic practice change—especially when multiple stakeholders or organizational resources are involved. Employ a multidisciplinary team when the change impacts several departments or requires varied expertise (e.g., infection control, pharmacy). --- 👀 Patterns to Recognize Repeated “trigger” language (e.g., “gap in practice,” “new guideline”) → opportunity for EBN project. Study designs in abstracts: Look for “randomized,” “controlled,” “cohort,” “case‑control” to quickly place evidence level. Outcome statements that include both statistical significance and clinical relevance → likely high‑impact evidence. Presence of patient‑centered language (values, preferences) → signals qualitative or mixed‑methods study. --- 🗂️ Exam Traps Distractor: “Level III evidence is superior to Level II because it’s non‑randomized.” Why wrong: Randomization (Level II) reduces bias; non‑randomized (Level III) is lower on the hierarchy. Distractor: “If a study is a systematic review, it automatically applies to all patients.” Why wrong: Applicability depends on patient similarity and context.
