Nursing Assessment
Introduction
Nursing assessment forms the foundation of nursing practice. It is a systematic and ongoing process through which nurses gather, organize, and analyze information about a patient to identify their health needs and determine appropriate nursing care. Unlike general health assessments that may focus on medical diagnosis alone, nursing assessment takes a broader, patient-centered approach that considers the whole person—their physical, emotional, social, and spiritual dimensions.
What Defines Nursing Assessment
Nursing assessment is distinguished by two key features:
Comprehensive Data Collection: Nurses collect information far beyond medical symptoms. This includes the patient's physical health status, functional abilities, psychological and emotional state, social support systems, cultural background, values, beliefs, and ability to manage their own health. A comprehensive nursing assessment paints a complete picture of the patient as a whole person, not just their illness or injury.
Patient-Centered Care Emphasis: Nursing assessment prioritizes the patient's perspective, preferences, and goals. Rather than simply identifying what is wrong from a clinical standpoint, nurses ask: What matters most to this patient? What are their concerns and priorities? What resources do they have? How do they prefer to be involved in decisions about their care? This patient-centered approach ensures that nursing care is individualized and responsive to what the patient actually needs and values.

Nursing assessment differs from medical assessment in important ways. While medical assessment primarily aims to identify disease and determine medical diagnosis, nursing assessment focuses on identifying actual or potential problems that nurses can help address—whether these involve managing symptoms, promoting independence, preventing complications, or supporting the patient through their health experience.
